Submit your completed immunization form.
The Board of Regents of the University System of Georgia (USG)
requires that each new student beginning at any USG institution
submit a Certificate of Immunization to the institution prior to
enrollment. Before you will be allowed to register for any
classes at Clayton State University, you must have this form
completed and returned to the Office of Recruitment and
Admissions. Failure to submit a completed Certificate of
Immunization will result in a registration hold on your student
record.
Your full immunization history must be transcribed onto this
form. Please contact your current healthcare provider to
transcribe your records; if your healthcare provider is unable
or unwilling to do so, our University Health Services will
transcribe your records or search the Georgia Registry of
Immunization Transactions and Services (GRITS) for a fee of
